Returning from a business trip in Hong Kong, Beth Emhoff has a layover in Chicago to have sex with a former lover. Two days later, in her family home in suburban Minneapolis, she collapses with seizures. Her husband, Mitch, rushes her to the hospital, but she dies of an unknown cause. Mitch returns home and finds that his stepson Clark has died from a similar disease. Mitch is put in isolation but is found to be immune; he is released and returns home to his teenage daughter Jory.

In Atlanta, representatives of the DHS meet with Dr. Ellis Cheever of the CDC and express fears that the disease is a bioweapon intended to cause terror over the Thanksgiving weekend. Cheever dispatches Dr. Erin Mears, an EIS officer, to Minneapolis to investigate. Mears traces the outbreak back to Beth. She negotiates with local bureaucrats, who are reluctant to commit resources for a public health response. Mears becomes infected and dies. As the virus spreads, Chicago is placed into quarantine and looting and violence breaks out.

At the CDC, Dr. Ally Hextall determines the virus is a mix of genetic material from pig and bat viruses. Work on a cure stalls because scientists cannot discover a cell culture within which to grow the newly identified MEV-1. UCSF professor Dr. Ian Sussman violates orders from Cheever to destroy his samples, and identifies a usable MEV-1 cell culture using bat cells. Hextall uses the breakthrough to work on a vaccine. Other scientists determine the virus is spread by fomites, with a basic reproduction number of four when the virus mutates; they project that one in 12 of the world population will be infected, with a 25-30% mortality rate.

Conspiracy theorist Alan Krumwiede posts videos about the virus on his blog. In one video, he claims he has cured himself of the virus using a homeopathic cure derived from forsythia. People seeking forsythia overwhelm pharmacies. During a television interview, Krumwiede discloses that Cheever secretly informed friends and family to leave Chicago before it was quarantined. Cheever is informed he will be investigated. Krumwiede, having faked his illness to boost sales of forsythia, is arrested for conspiracy and securities fraud.

Using an attenuated virus, Hextall identifies a possible vaccine. To cut out the lengthy time it would take to obtain informed consent from infected patients, Hextall inoculates herself with the experimental vaccine and visits her infected father. She does not contract MEV-1 and the vaccine is declared a success. The CDC awards vaccinations by lottery based on birthdate. By this time, the death toll has reached 2.5 million in the U.S. and 26 million worldwide.

In Hong Kong, WHO epidemiologist Dr. Leonora Orantes and public health officials identify Beth as the index case. Government official Sun Feng kidnaps Orantes to use her as leverage to obtain MEV-1 vaccine doses for his village. WHO officials provide them with vaccines and she is released. When Leonora learns the vaccines given to the village were placebos, she runs to warn them.

In China, days before Beth is infected, a bulldozer knocks down a tree, disturbing some bats. One flies over a pig pen and drops a piece of banana, which is eaten by a pig. The pigs are slaughtered and prepared by a chef who shakes hands with Beth in the casino, transferring the virus to her.

In 1995, Chuck Noland (Tom Hanks) is a time-obsessed systems analyst, who travels worldwide resolving productivity problems at FedEx depots. He is in a long-term relationship with Kelly Frears (Helen Hunt), with whom he lives in Memphis, Tennessee. Although the couple want to get married, Chuck's busy schedule interferes with their relationship. A Christmas with relatives is interrupted by Chuck being summoned to resolve a problem in Malaysia.

Flying through a violent storm, his airplane crashes into the Pacific Ocean. Chuck is able to escape the sinking plane and is saved by an inflatable life-raft but in the process, loses the raft's emergency locator transmitter. He clings to the life-raft, loses consciousness, and floats all night before being washed up on an island.

After he awakens, he explores the island and soon discovers that it is uninhabited. Several FedEx packages from the crashed plane wash up on the shore, as well as the corpse of one of the pilots (whom he buries). He initially tries to signal for rescue and makes an escape attempt with the remnants of his life-raft, but he cannot pass the powerful surf. He searches for food, water, shelter, and opens the packages, finding a number of potentially useful items. He leaves one package, with a pair of wings painted on it, unopened.

During a first attempt to make fire, Chuck receives a deep wound to his hand. In anger he throws several objects, including a Wilson Sporting Goods volleyball from one of the packages. A short time later he draws a face in the bloody hand print on the ball, names it Wilson and begins talking to it.

Four years later Chuck is dramatically thinner, bearded, his hair is longer, and he is wearing a loincloth. He has become adept at spearing fish and making fires. He also has regular conversations and arguments with Wilson.

After a large section from a portable toilet washes up on the island, Chuck uses it as a sail in the construction of a raft. After spending some time building and stocking the raft and deciding when the weather conditions will be optimal (using an analemma he has created in his cave to monitor the time of year), he launches, using the sail to overcome the powerful surf. After some time on the ocean, a storm nearly tears his raft apart. The following day, "Wilson" falls from the raft and is lost, leaving Chuck overwhelmed by loneliness. Later, he is found drifting by a passing cargo ship.

Upon returning to civilization, Chuck learns that he has long been given up for dead; his family and friends held a funeral, and Kelly has since married Chuck's dentist and has a daughter. After reuniting with Kelly, the pair profess their love for each other but, realizing a future together would be impossible due to her commitment to her family, they part. Kelly gives Chuck the keys to the car they once shared.

Chuck then travels out into the country to return the unopened FedEx package to its sender. The house at the address is empty, so he leaves the package at the door with a note saying that the package saved his life. He then departs and stops at a remote crossroads. A woman passing by in a pickup truck stops to explain where each road leads. As she drives away, Chuck notices the illustration on her truck is similar to the one on the parcel. Chuck is left looking down each road. The camera focuses on his face as he looks toward the departing woman in the truck.

In 2002, Bolivian politician Pedro Castillo (a fictionalized version of Gonzalo Sánchez de Lozada) hires an American political consulting firm (based on James Carville's Greenberg Carville Shrum firm) to help him win the 2002 Bolivian presidential election. The firm brings in Jane Bodine (Sandra Bullock) to manage the campaign in Bolivia. The opposition's political consultant is her nemesis, fellow American Pat Candy (Billy Bob Thornton). In Bolivia the situation is tense: the indigenous people, who are a majority in the country, are protesting for a constitutional reform, to get proper representation.

The American consultants, not knowing the language or the culture, are persuaded by Bodine, a burned out political strategist, to follow a strategy of smear campaigning. However, Castillo refuses to give permission for the team to do so. It is only after publishing a flyer accusing Castillo of a long ago affair (and blaming it on the opposition) does Castillo agree to smear his opponents likewise.

In the following months, the team exercises a strategy of declaring a crisis. They are planning on frightening the people, so they would be persuaded to vote for the unsympathetic but known Castillo rather than the younger oppositions' candidates. Also they publish photos of their enemy with Nazi war criminal Klaus Barbie in the background, so that he has to deny being a Nazi. Castillo's bus is stopped by a group of protesters who don't want the International Monetary Fund in Bolivia. Castillo promises not to invite the IMF without a referendum. Eduardo, a young volunteer of the Castillo campaign, is deeply impressed by this. His loyalty comes mostly from the fact that Castillo, who was President at the time, took Eduardo on his arm, during a rally in his town. Nevertheless, his brothers are much more skeptical about Castillo.

During the final debate, Bodine cites a quote in her conversation with Candy, which he gives to Rivera in his speech, saying that "a great man" said it. Unfortunately the quote is actually from Joseph Goebbels, Adolf Hitler's minister of propaganda. Castillo wins the vote by a small margin. As one of his first actions, he invites the IMF, thereby breaking his promise. The deeply disappointed Eduardo visits Bodine in her hotel, who says that she is not responsible for Castillo's actions. In her eyes, her job is done.

The disillusioned Eduardo joins his brothers at a demonstration of people demanding change. The police arrive and the demonstration quickly turns into a riot. Bodine and her crew are with Candy on the way to the airport. All of them, except Bodine, already have jobs as political consultants in other countries. When Bodine realizes that she brought a liar into office, she leaves the car. At the demonstration she meets Eduardo. In a final clip of the TV interview she has been shown giving throughout the movie, it's revealed that she is now the head of an organization that promotes Latin American solidarity.

In 1957 New York City, Rudolf Abel is arrested and charged with spying for the Soviet Union. Insurance lawyer James B. Donovan is appointed to defend Abel, so that Abel's trial will be seen as fair. Committed to the principle that the accused deserves a vigorous defense, he mounts the best defense of Abel he can, declining along the way to cooperate in the CIA's attempts to induce him to violate the confidentiality of his communications with his client.

Abel is convicted, but Donovan convinces the judge to spare Abel the death penalty because Abel had been serving his country honorably, and he might prove useful for a future prisoner exchange; Abel is sentenced to 30 years. Donovan appeals the conviction to the Supreme Court based on the lack of a search warrant for the seizure of Abel's ciphers and photography equipment. For his principled stand Donovan and his family are harassed, including shots being fired at their home. The conviction is upheld.

In 1960, Gary Powers, a pilot in the CIA's top secret U-2 spy plane program, is shot down over the USSR. He is captured and sentenced in a show trial to ten years confinement, including three years in prison.

Donovan receives a letter from East Germany, purportedly sent by Abel's wife, thanking him and urging him to get in contact with their lawyer, whose name is Vogel. The CIA think this is a back-channel message hinting that the USSR is willing to swap Powers for Abel. They unofficially ask Donovan to go to Berlin to negotiate the exchange; he arrives just as the Berlin Wall is going up. Crossing in to East Berlin, he meets with a KGB officer in the Soviet Embassy and is then directed to Vogel, who represents the Attorney General of the German Democratic Republic (GDR) (East Germany). The Attorney General seeks to swap Abel for an American graduate student named Frederic Pryor, who had been arrested in East Germany; in the process the GDR hopes to gain official recognition by the United States.

The CIA wants Donovan to disregard Pryor but he insists that both Pryor and Powers be swapped for Abel. In a message to the Attorney General he bluffs that they will either release Pryor with Powers or there will be no deal. The exchange of Powers and Abel takes place at the Glienicke Bridge, with Pryor to be released simultaneously at Checkpoint Charlie. Tension builds as Pryor fails to arrive. The CIA, still primarily concerned with Powers, tell Abel he can go, but he refuses to move. It is confirmed that Pryor has been released, and the exchange takes place.

The next day, back in the United States, the government publicly acknowledges Donovan for negotiating the deal that rehabilitates his public image.

Jesper (Schwartzman) comes from a wealthy family in the postal business, and is a selfish brat with no life ambitions whatsoever. When Jesper's father puts him in the royal postal academy in an attempt to teach him that hard work pays and being from a rich family is not a shoo in to wealth, he deliberately distinguishes himself to be the academy’s worst student, and so his father comes up with another plan to teach him a lesson: he is stationed on a island above the Arctic Circle by his father, with the ultimatum that if he doesn't post 6,000 letters in a year, he will be cut off from the family.

He gets into the island's town of Smeerensburg and is shown around by a sarcastic ferry boat skipper who tricks him into ringing a bell to start the reception, instead revealing that the town inhabitants hardly exchange words let alone letters; they are divided, feuding locals filled with anger, bitterness, hatred and animosity. Trying desperately to come up with a way to get the town locals to send letters, he notices on the map in his office a far off little establishment. Investigating, he finds a woodsman named Klaus (Simmons), with a skill of woodworking and a house with lots of handmade toys.

Jesper is just about to finally give up when unexpected events unfold that give him the idea of having Klaus donate the toys in his house to the town kids who would ask him for them, and who, by doing so, would send letters. When Jesper goes to Klaus with his proposal of donating the toys, he agrees, provided the deliveries will be at night, so he can accompany Jesper on them. When Jesper finds out that many of the kids can't write, he tells them they can simply learn at school, and so they go to Ms Alva (Jones), a qualified teacher who Jesper met back on his first day in town, to learn how. The increasingly developing actions of Jesper and Klaus delivering of toys becomes the talk of the town kids, with the nature of said actions making them believe Klaus is not only solely responsible, but he also has certain quirks. Some of these quirks are also said to be magic, such as he can enter homes through any chimney and can never be seen: the most astounding of all is he has a reindeer-pulled sleigh that can fly. When Jesper says to the kids that Klaus would not give toys to bad kids, he goes on to say Klaus always knows whenever any kid is naughty, and their attitude changes, even the town bully, as he also wants Klaus' toys. Their resulting acts of kindness inspire the other townsfolk to do the same.

Jesper eventually finds out Klaus lost his wife and they could never have children, but he loves making kids happy and had made all the toys in his house for the ones he thought they would have. They soon receive help from a small community of kind Sámi people to fulfill Jesper's prior idea of a delivery run to give toys to the kids on Christmas, but not only that, Jesper begins to change himself, no longer being selfish.

While all of this has been happening, the heads of the town's feuding families, through the families' clearly inextinguishable, and thus, also, very long-standing hatred to each other, have been trying to stop Jesper and Klaus to preserve this tradition, and they agree on joining forces to do it. They eventually come up with a plan that involves making Jesper leave by using his past against him, but he ultimately makes his decision final to stay in Smeerensburg, and proceeds to do everything that he can to stop the rest of their plan. Though he finds out Klaus and Alva already knew what was coming and had made preparations to foil the plan, his actions cause events that make the families find themselves as in-laws.

As things in Smeerensburg keep getting better, Jesper and Klaus continue delivering presents on Christmas, with their operation expanding further and further as time continues on. Then, on the twelfth year, Klaus suddenly disappears without a trace, joining his departed wife. Jesper and Alva get married and have two children together, and every Christmas Eve, Jesper gets to see the spirit of his friend, as he continues to deliver toys to kids around the world.
